Billing Change Request Form
NOTE: Any Youth Program terminations, freezes, or comments should be directed to the Youth Programs email (find your gym's YP email below).
Please read carefully before proceeding:
- Cancellation requests must be received by the 25th of the month to be in effect for the upcoming month
- Freeze requests must be received by the 25th of the month to be in effect for the upcoming month
- Memberships can be frozen for $10.00 per month per member*
- Monthly freeze fees may be waived for medical reasons; medical documentation must be sent to [email protected] before the 1st of the month
- If your membership is frozen for medical reasons; you will not be permitted to use the facilities until your membership is either unfrozen or terminated
- Please allow up to 5 business days for any response to your billing inquiry
For Youth Program Members:
- Cancellation requests must be submitted one calendar month before you wish to leave the team (ex: if you wish to leave in March, you must submit your request before February 1st)
- Unless you have a medical reason (see above for policy of medical freezes), we do not approve freezes. However, if you wish to rejoin the team after cancelling, the enrollment fee is reduced to $39
